Box 1. Payroll information is correct and complete; wage and fringe rates are not less than the applicable wage determination. Box 2. All required basic records are complete, accurate, and available on request. Box 3. Classifications reported match the work each worker actually performed. Box 4. I paid registered apprentices this week, in programs registered with OA or a State Apprenticeship Agency. Leave this unchecked if no apprentices were paid; the PDF enters N/A for you. Box 5. Fringe benefits were paid to bona fide plans and/or in cash as reported. Check this when you take fringe credit for plans or pay fringes as cash. Box 6. All workers were paid full weekly wages with only permissible deductions (29 CFR part 3).

Check each statement only if it is true: you, not this tool, are certifying. DOL's instructions say boxes 1, 2, 3 and 6 must always be checked. Willful falsification is a federal crime (18 U.S.C. § 1001).
